​​What is the Pre-Treatment Questionnaire?

The questionnaire is to ensure that Sports Massage Therapy is an appropriate treatment at the time, and obtain patient consent to proceed with the treatment.  There are some conditions for which you may be advised to consult with your GP/medical practitioner ahead of receiving massage treatment.  

 

Please do not complete the questionnaire ahead of time as it must be completed at your appointment - the questionnaire will be provided for you. 

Your Sports Massage Therapist will then review your completed questionnaire before proceeding with any treatment.  Please be aware that, if appropriate, they may refer you to your GP/medical practitioner for advice.
